    Hair transplant surgery
I am of Pakistani origin, and I recently visited Lahore on holiday. One thing that I could not help but notice was the vast amount of billboards advertising men’s hair transplant surgery.
Baldness is extremely common, as it affects over 60% of men and 10% of women. It is thought to be caused by an autosomal dominant gene, hormonal changes, illness or medications.
Hair transplant surgery is placing new hair follicles to fill an area where the hair has stopped growing or is thinning. This is done by taking hair from other parts of the body and usually placing it on the front and sides of the head. Doctors have been carrying out this procedure in the USA since the 1950s, but it is originally thought to have started in Japan.
Hair transplantation is based on the theory of donor dominance. If hair is taken from an occipital donor area and transplanted to a frontal recipient area, it is still able to survive and grow without being attacked by the immune system (alopecia areata).
There are 2 main types of hair transplants: FUT (Follicle Unit Transplantation) and FUE (Follicle Unit Extraction). FUT involves dissecting of part of the scalp on the back of the head into individual grafts, and FUE involves removing individual hairs from the occipital region of the head and placing it into the balding areas. I personally feel that FUE is a better option as the patient will have fewer cuts, stiches and scars. However, in FUT, larger areas of baldness can be covered.
I shall now describe the procedure of FUE.
Firstly, the person’s hair is assessed to see what type of hair should transplanted into the area and how well the hair will grow after the operation. Also, the less the contrast in colour between the scalp and the hair, the better the results will be, as the transplant will look less obvious.
The patient is put under local anaesthesia and the head is sterilised.
The next step is the most important: planning the hairline. This is the most important landmark of a hair transplant, and the result of this operation is often judged by how natural and appealing the hairline looks. The hairline should start approximately 8cm from the glabella.
The hairline is created by 250-300 single hair grafts. The grafts are inserted into small slits or pores using forceps. They are placed in a saw-tooth pattern to create a natural look. Some surgeons use punch grafts to remove circular areas of tissue where the hair follicles can be placed.
After the operation, the head can be swollen and very tender, so the patient may have to take pain-relief medication. The transplanted hairs start to grow immediately, and the hair is fully grown after about 6 months.
Compared to other cosmetic operations, there are few post-operative complications. These include bleeding or infection. The pros of hair transplantation is that the patient will have increased self-esteem and long-term results.
But why is hair transplant surgery so popular for men in Pakistan and India? These countries have some of the world’s best cosmetic surgeons, who offer the procedure at a slightly cheaper price. Furthermore, research shows that the reason more people in India and Pakistan are balding is mainly due to inherited factors, as well as hormonal changes, iron deficiency, lack of protein in diet, lack of vitamin D, stress and related diseases.
